Ascend Personal Care Services LLC provides non-medical personal care services designed to support individuals who wish to remain safely and comfortably in their homes.
Our services focus on assistance with activities of daily living and are delivered in accordance with Virginia Department of Health Home Care Organization requirements.
Professional Oversight
Personal care services are delivered under the supervision of a Virginia-licensed Registered Nurse.
The Registered Nurse conducts initial assessments, develops individualized plans of care, and provides ongoing oversight to ensure services remain appropriate and consistent with state requirements.
Service Scope Notice
Ascend Personal Care Services LLC does NOT provide skilled nursing services, therapy services, or medical treatment.
Services are limited to non-medical personal care as permitted under Virginia Department of Health regulations.
