Our care process is designed to ensure that personal care services are appropriate, well-planned, and delivered with consistency.

Each step emphasizes clear communication, professional oversight, and compliance with Virginia Department of Health Home Care Organization requirements.

Step 1:
Initial Inquiry

Families, clients, or referral partners may contact us to learn more about our personal care services, service area, and general approach to care. During this initial conversation, we gather basic information to understand needs and determine whether our services may be appropriate.

Step 2:
Registered Nurse Assessment

Before services begin, a Virginia-licensed Registered Nurse conducts an initial assessment to evaluate functional needs, safety considerations, and appropriateness for personal care services. This assessment helps ensure services are suitable and aligned with applicable regulations.

Step 3:
Individualized Plan of Care

Based on the registered nurse’s assessment, an individualized plan of care is developed. The plan outlines the personal care services to be provided, frequency of services, and any specific instructions needed to support safety and consistency. Services are limited to non-medical personal care and assistance with activities of daily living.

Step 4:
Ongoing Supervision & Review

Personal care services are delivered in accordance with the plan of care and are overseen by a registered nurse. Services are reviewed at regular intervals or when needs change to ensure continued appropriateness and quality.
